About us
We are a Direct Primary Care clinic providing patient-centered care by increasing access to your family medicine doctor.
Our visits are unhurried and typically last for about one hour which allows the doctor-patient relationship to grow organically. These visits are by appointment only.
Because our membership is limited to a finite number of patients, same or next-day appointments are available.
We do not accept insurance so there is no middle-man; the only interaction is between you and your doctor.
Communication will be via e-mail or phone during office hours. After hours correspondence will be answered by the next business day.
Who We Are
Your care is led by Dr. Zena Joseph who specializes in Family Medicine. She is a graduate of UPMC St. Margaret and has worked as an attending physician on Long Island since 2012. She has held various positions in the community such as faculty at Stony Brook Medical Center and was the consulting physician for Longwood School District.
She is located in the heart of Port Jefferson just steps away from the ferry.
Visits with Dr. Joseph are by appointment only.

Direct Primary Care (DPC) is a healthcare model in which patients pay a monthly fee directly to their primary care physician for a range of services bypassing traditional health insurance billing. It allows for doctors to have personalized, longer visits with their patients and eliminates a third-party interfering with your care.

No, we do not accept health insurance.

Our in-office hours are from 9:30am until 3:30pm from Monday through Thursday and we are available by phone from 8am until 7pm from Monday through Friday.

In order to ensure that each patient has the appropriate amount of time with the physician, we do not take walk-in appointments.

Patients will pay a monthly membership fee which will be billed on the first of the month. Please call us at 631-683-8148 to discuss pricing options.

No, we do not prescribe controlled medications.

Membership includes open access to your doctor via in-person appointments or telehealth/phone visits (for patients residing in the state of New York) during office hours Monday through Friday.

No! Feel free to cancel your membership at any time. We ask that you just give us 30 days notice so we can ensure that your prescriptions are covered for at least a month. Please note that there is a re-enrollment fee of $600

We will be able to give lab and imaging requisition forms to you in order to have appropriate testing done at participating facilities. You can still use your health insurance to pay for these tests.

No, there is no enrollment fee to join! Please note that there is a re-enrollment fee of $600 if you terminate your membership and then decide to rejoin the practice.
